Frequently Asked Questions

Here are the most frequently asked questions to help you get started.

To properly triage your patient, we need a referral that contains the diagnosis and/or suspected diagnosis. We will also need the following information with each referral:

  • Demographic sheet on the patient that includes a copy of their insurance cards or detailed insurance information
  • Clinical documentation supporting the referral such as lab testing, radiology testing, chart notes, etc.
  • If your patient is already on treatment or an infusion, please send over the last infusion/injection flowsheet.

Referrals are sent to our New Patient Coordinators who first verify the information in your referral. The information to contact the patient is then loaded into our referral tracking system and calls start going out to your patient(s). Please make sure your patients are aware that they will receive an automated call to schedule their new patient appointment and it may show up as an 800 number. 

The patient will then be scheduled according to referral reason and location or provider. If your patient has an HMO policy or one that requires a PCP referral, our NP Coordinators will request the PCP referral from your office (if you are the PCP) or the patient’s PCP if you are their specialist.
